Teams from Puerto Rico are eligible to play in the ASICS®/Tampa Bay’s BIG SOUTH® National Qualifier providing they complete the necessary requirements. Please review the following list and make sure to complete all 8 steps.

1. Click here to download the USAV required Player Registration Form. You must have this 2 page form filled out and signed by EVERY PERSON listed on your roster -- players, coaches, chaperones, etc.
2. Click here to download the USAV required Medical Release Form.  You must have this form filled out and signed by EVERY PLAYER listed on your roster.
3. Click here to download the USAV summary of the Code of Conduct.  This form is a copy of what is included in the Player Registration Form.  You should make sure that each player and coach has a copy of this form to keep for their records. Do not turn this form in to the tournament.
4. When you check your team in at the tournament, you must have a government photo ID for EVERY PERSON listed on the roster to show to the event registration staff.
5. When you check your team in at the tournament, you must have a birth certificate for EVERY PLAYER listed on the roster to show to the event registration staff.
6. When you check your team in at the tournament, you must have proof or verification of your team and/or club’s registration with the Puerto Rican National Volleyball Federation. (This can be a letter from your Federation)
7. When you check your team in at the tournament, you must pay a $100 per team fee for USA Volleyball Foreign Athlete Insurance. This fee can be paid in cash or in a money order ONLY.  No checks or credit card will be accepted.
8. MOST IMPORTANT -- Go to the link to the left or “How to Enter” and follow all instructions for getting your team entered correctly and registered for the Qualifier.
