If you are interested in attending a clinic for IMPACT certification, the Southern Region will be offering a clinic at the event. Please click on the link for the Registration Form and send in right away.
REGISTRATION DEADLINE is seven [7] days prior to the clinic. NO REFUNDS will be granted for withdrawals after the registration deadline. LATE REGISTRATIONS will only be accepted at the discretion of SRVA. There will be no registrations accepted at the clinic, and only those who’ve registered may attend.
PLEASE NOTE: SRVA may CANCEL the CLINIC for lack of registration, bad weather, etc., and refund registration fees.
COST:
$25 PER SRVA MEMBER [if received by registration deadline]
$50 PER NON-SRVA MEMBER [if received by registration deadline]
$25 LATE FEE [if late registration is accepted]
MAKE CHECKS PAYABLE TO SRVA. SEND COMPLETED REGISTRATION FORMS and PAYMENT TO: SRVA, P. O. BOX 19425, BIRMINGHAM, ALABAMA 35219.
